心肾综合征住院患者危险因素和生物标志物的预后意义:一项初步研究

背景:心肾综合征(CRS)涉及心肾串扰以及神经体液和炎症通路的激活,是一种具有高发病率和高死亡率的疾病。 目的:评价危险因素和生物标志物在CRS住院患者预后中的作用。 方法:在这项观察性队列研究中,100名连续住院的CRS患者被纳入研究。入院时记录社会人口学特征、个人病史和既往用药情况,并进行超声心动图检查。此外,还测量了一系列血液标志物。研究终点为出院时死亡或透析依赖的复合指标。结果:患者分为两组;组1 (N= 52):出院时不依赖透析;组2 (N=48):出院时死亡/透析依赖。两组间基线特征无显著差异。2组患者较少使用肾素-血管紧张素-醛固酮系统阻滞剂(RAASb),而较多出现少尿/无尿。2组患者血红蛋白、血清白蛋白、25-羟基维生素D (25(OH)D)明显降低。与此同时,2组患者血清磷酸、钾、甲状旁腺激素(PTH)显著升高。在一项多元回归分析中,缺乏先前的RAASb和较低的25(OH)D水平与出院时死亡风险增加或透析依赖独立相关。25(OH)D/PTH比值是复合终点最准确的预测因子(敏感性:79.4%,特异性:70.4%)。 结论:既往未使用RAASb、高甲状旁腺激素、低25(OH)D水平和低25(OH)D/甲状旁腺激素比值与CRS住院患者预后不良相关。
