摘要
炎症性肠病(IBD)是一种病理机制复杂的肠道疾病,包括克罗恩病和溃疡性结肠炎。近年来,IBD的发病率呈上升趋势,对患者的生活产生了显著的负面影响。因此,寻找新的治疗靶点和创新药物对IBD的发展尤为重要。近期研究表明,NLRP3炎性囊泡在IBD中维持肠道稳态和维持肠道免疫应答中发挥重要作用。一方面,NLRP3炎性囊泡的异常激活可能通过将caspase-1、proIL-18和proIL-1β转化为活性形式,并释放促炎细胞因子,刺激IBD的发生和进展,从而引起过度的免疫反应,我们可以通过靶向阻断NLRP3激活来改善IBD。另一方面,NLRP3也可能通过维持肠道免疫系统的稳态发挥进入保护作用。本文综述了NLRP3炎性小体的激活机制,并从病理和保护两个角度探讨了NLRP3炎性小体激活对IBD的作用。同时,我们结合前沿研究进展和临床实践结果,列出了NLRP3炎性体的直接抑制剂、间接抑制剂和天然抑制剂对IBD的作用,为IBD的临床治疗提供了新的靶点和新的思路。
